The most reliable way is to measure a shirt you already own that fits you well. Lay it flat without stretching it:

Chest: measure from armpit to armpit and multiply by two. Compare the result with the "Chest" column.
Length: measure from the highest point of the shoulder, next to the collar, down to the hem.
Kids: go by height; age is only a guide.
Trainers: stand with your heel against a wall on a sheet of paper, mark the tip of your longest toe and measure the distance in centimetres.
